Procedure to follow

Please contact the desired centre if you would like dialysis in 1 of these 3 centres. Please give the dates of the dialysis sessions and ask to speak to the nephrologist present.

You should then fill in the medical dossier available here and return it by email or fax (to the number +32 2 614 98 17) to the nephrologist you contacted. You should give the date of the last dialysis session in your centre, the required date of the first dialysis session in our centre, the required period of dialysis and the date of the last dialysis session with us. We will respond as quickly as possible.

If you have an insurance policy/sickness insurance fund that insures reimbursement for dialysis in Belgium, you should have these documents available (European health insurance card (EZVK), form S2 => E112). If you do not have any of these cards/forms you will be asked to pay for the costs of each dialysis beforehand (about 400 euros per dialysis). On the day before or on the day of the first dialysis you should go to register at the administrative department, stating that you have come for dialysis: you should ask for Mrs Arquin (St-Elisabeth site) or for Mrs Willems (St-Michel site).
